Job Description: The home health aide provides personal care and related services in the home that are allowed to be performed under state law. She/he functions under the direction, instruction and supervision of the staff nurse and the Director of Skilled Services and/or appropriate supervisor.

Qualifications: Must be able to speak, read, write & communicate effectively in English and meet one of the following qualifications:

  1. Satisfactory completion of a nursing education program preparing for RN/LPN licensure
  2. Satisfactory completion of a nurse aide education program approved by the VA Board of Nursing
  3. Have certification as a nurse aide issued by the VA Board of Nursing
  4. Be successfully enrolled in a nursing education program preparing for RN or LPN licensure and have currently competed at least one nursing course that includes clinical experience involving direct client care
  5. Have satisfactorily completed training & passed a competency evaluation program that meets the criteria of CMS CoPs- 42 CFR 484.80.
    Must have a criminal background check and current CPR certification.
    Prefer 1 year experience as an Aide in a health related setting, preferably home health

Responsibilities: Tasks to be performed by an HA must be assigned by and performed under the supervision of an RN who will be responsible for the Client care provided by the HA. Perform services that are permitted under state law. Duties of the HA may include:

  • Assist with ADLS and iADLs as per the POC
  • Follow the POC for each Client and document the same
  • Help Clients to maintain good personal hygiene
  • Assist in maintaining a healthful, safe environment
  • Plan/prepare nutritious meals
  • Markets when indicated on the POC
  • Assist Clients with ambulation as per the POC
  • Assist with certain treatments as directed by the Skilled nurse
  • Assist the therapy personnel as needed with rehabilitative processes per the HA POC
  • May cue for medications ordinarily self-administered as assigned
  • Taking and charting vital signs under the direction of a nurse or healthcare practitioner
  • Encourages Clients to be as independent as possible according to the nursing care plan
  • Attempts to promote Client’s mental alertness through involvement in activities
  • Gives simple emotional support to Clients and other members of the household
    Establishes a relationship with Client and family which transmits trust and confidentiality
  • Prepares a report of visits on the day it is performed and incorporates same in the clinical record weekly or as directed

Job Description: A Licensed Practical Nurse provides practical nursing skills within the scope of nursing practice standards in collaboration with RN case managers/supervisors, teaches and educates Clients and their families according to the established professional plan of care. Licensed Practical nursing is performed under the supervision of the assigned RN’s, Administrator, and Director of Skilled Services.

Qualifications:

  1. Graduate of an approved LPN nursing school
  2. Currently, licensed “in good standing” in VA
  3. A minimum of one (1) year of experience as a LPN, preferably in a home care setting
  4. Must have a criminal background check and current CPR certification.

Responsibilities:

  • Maintains the nursing frequency established by the supervising RN.
  • Prepares equipment & materials for treatments observing aseptic techniques as required.
  • Follows the plan of care established by the supervising RN.
  • Performs home safety check and environmental check of the Client’s home environment on all visits.
  • Reports any unsafe or potentially unsafe condition to the supervising RN.
  • Performs nursing visit and documents accordingly.
  • Reviews and follows the supervising RN’s plan of care, communicating frequently with the supervising RN on any suggested changes necessary.
  • Collaborates with the supervising RN on a weekly (or more frequent basis) to benchmark toward the established outcome goals.
  • Reviews the Client’s ADL and iADLs abilities.
  • Teaching clients & family self-care techniques.
  • Communicates any suggested changes to the supervising RN.
  • Reinforces the HA plan of care when HA services are ordered.
  • Effectively communicates with Client/family the POC and progression of such.
  • Effectively communicates with other disciplines in the case (actively participates in case conferencing) to effectively and appropriately problem solve as situations arise.
  • Communicates effectively with the supervising RN, scheduled visits planned and changes to the schedule on a weekly basis.
  • Caseload is self-scheduled but communication of the clinician’s schedule to the nursing supervisor is essential.
  • Communicates in case conferencing sessions. Effectively functioning as an involved team member to collaborate on best practices for the individual Client’s needs.
  • Submits accurate, complete paperwork at the end of every week so that all medical records are intact and up to date. Uses the drop box if the office is already closed for the weekend.
  • Notifies the supervising RN with requests for re-authorization of “more visits needed” prior to third party insurers authorized number of visits expiring.
  • Suggests Community Services that may be available to the Client to assist in safe home care needs.
  • Participates in the QI Committee
  • Participates in staff meetings. Adjusts visits on the staff meeting day to accommodate this function.

Job Description: An OT provides professional services within the scope of occupational therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates Clients and their families. OT in the home is performed under the supervision of the RN case manager, Administrator, DSS. If Nursing has opened the case, Occupational Therapy is a secondary discipline and therefore works collaboratively and professionally with the RN case
manager to provide necessary services.

Qualifications:

  • Graduated after successful completion of an OT education Program accredited by the Accreditation
  • Council for OT Education (ACOTE) of the American OT Association (AOTA) or successor organizations and is eligible to take or has successfully completed the entry level certification exam for OTs from the NBCOT.
  • A currently licensed in VA.
  • Will have a minimum of two (2) year experience as a OT, preferably in a homecare setting.
  • Criminal check & Current CPR certification.

If educated outside of the United States must meet both of the following: Graduated after successful completion of an OT education program accredited as substantially equivalent to OT entry level education in the US by one of the following:

  • Accreditation Council for OT Education (ACOTE), successor organizations of ACOTE, the World Federation of OTs, a credentialing body approved by the American OT Association, successfully completed the entry level certification exam for OTs developed & administered by the National Board for Certification in OCC Therapy (NBCOT).
  • On or before December 31, 2009, is licensed or otherwise regulated, if applicable, as an OT by the state in which practicing.

Responsibilities:

  • Receives intake referral information and prepares OT paperwork/tools necessary for the visit.
  • Continuing OASIS data sets. Obtains pertinent medical history from Client, family or significant others.
  • Performs home safety check and environmental assessment of the Client’s home environment.
  • Performs OT evaluation or skilled visit and documents accordingly.
  • Performs physical examination and review of all body systems and documents such accordingly.
  • Develops an appropriate and effective Plan of Care (POC) to be submitted to the physician for approval and implementation.
  • Evaluates the Client’s ADL and iADLs abilities.
  • Assists the physician in evaluating level of function.
  • Functionally reassesses the Client for on-going therapy services.
  • Helps develop the plan of care (revising if necessary) and at least every 30 days.
  • Develops and implements the HA plan of care when HA services are ordered.
  • Revises and signs this care plan the beginning of each certification period if nursing has already discharged out of the case.

Job Description: A Physical Therapist provides professional services within the scope of physical therapy practice standards in collaboration with primary care physicians and other members of the health care team and teaches and educates Clients and their families.

Qualifications:

  • Successful completion & graduate of a PT Program approved by the Commission on Accreditation in PT education (CAPTE) or successor organizations of the CAPTE or an education program outside of the United States determined to be substantially equivalent to physical therapist entry level education in the US by a credentials eval organization approved by the American PT
  • Association or an organization identified in 8 CFR 212.15(e) as it relates to physical therapists.
  • A current, active license to practice Physical Therapy in VA.
  • A minimum of two (2) years’ experience as a Physical Therapist.
  • Criminal Check & Current CPR certification.

If trained outside of the United States before January 1, 2008, meets the following requirements:

  • Was graduated since 1928 from a PT curriculum approved in the country in which the curriculum was located and in which there is a member organization of the World Conference for Physical Therapy.
  • Meets the requirements for membership in a membership organization of the World Confederation for Physical Therapy.

Responsibilities:

  • Obtains all pertinent medical history from Client, family or significant others.
  • Performs PT assessment visit and documents accordingly.
  • Performs home safety check and environmental assessment of the Client’s home environment.
  • Assists the physician in evaluating the level of function.
  • Helps develop an appropriate and effective PT Plan of Care (POC) to be submitted to the physician for approval and implementation, revising as necessary and at least every 30 days.
  • Functionally reassess the Client for on-going therapy.
  • Determines medical necessity for other services.
  • Evaluates the Client’s ADL and iADLs abilities.
  • Develops and implements the HA plan of care when HA services are ordered and PT is the only skilled discipline.
  • Supervises the HA in accordance with state/federal requirements and documents the supervision without having to be directed to do so, when PT is the only skilled discipline. Effectively communicates ongoing with Client and family regarding the POC and progression of such.
  • Effectively instructs Clients and families in the safe use of prosthetic, orthotic, and assistive devices (canes, walkers, wheelchairs, mechanical lifts etc.).
  • Effectively communicates with other disciplines in the case.
  • Communication with the Client’s physician (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities.

Job Description: A professional who provides professional nursing services within the scope of nursing practice standards in collaboration with primary care physicians, teaches and educates Clients and their families. Professional skilled nursing is performed under the supervision of the Administrator/Director of Skilled Services.

Qualifications:

  • Will hold an active license to practice professional nursing in VA.
  • Will have a minimum of one (1) year experience as a professional nurse.
  • Preferably has completed a BA degree program approved by the National League for Nursing for Public Health Nurses.
  • Must have a criminal background check.
  • Must have current CPR certification.

Responsibilities:

  • Furnishes those services requiring substantial and specialized nursing skill.
  • Initiates appropriate preventative and rehab nursing procedures.
  • Prepares & submits timely, clinical and progress notes.
  • Oversees coordination of Client services.
  • Informs MD and other staff of changes in PT situation or needs.
  • Completes medical history/home safety check/environmental assessments.
  • Performs socio-psychological evaluation.
  • Performs assessment visit and documents timely. (i.e.. OASIS/ Skilled Nursing Note etc.).
  • Performs physical examination and review of all body systems and documents accordingly.
  • Develops individualized Plan of Care (POC) to be submitted to the physician for approval and implementation and performs necessary revisions based upon Client needs.
  • Determines medical necessity for other services.
  • Regularly re-evaluates the Client’s nursing needs.
  • Evaluates the Client’s ADL and iADLs abilities and need for home health aide.
  • Develops and implements the HA plan of care when HA services are ordered.
  • Revises and signs this care plan the beginning of each certification period.
  • Supervises HA in accordance with state/federal requirements and documents the supervision.
  • Orders “other” professional services that may be appropriate to the needs.
  • Reviews billing processes with Client and/or family advising Client and/or family when co-pay or Medicare is not likely to pay for services.
  • Effectively communicates ongoing with Client and family progression/changes in POC.
  • Effectively communicates with Supervisor/Nursing Director/other disciplines in the case.
  • Communication with the Client’s MD (verbally and/or in writing) to obtain effective treatment modalities and/or rehabilitative therapy modalities.
  • Actively communicates in the case conferencing sessions to establish best practices.
  • Submits requests for re-authorization of “more visits needed” to payers timely.
  • Coordinates Community Services for the Client to assist in safe home care needs.
  • Participates in the QI Committee.

Job Description: A Speech Pathologist/Therapist provides professional services within the scope of Speech Pathologist practice standards in collaboration with primary care physicians, teaches and educates Clients and their families, and evaluates and assesses Client’s therapy needs.

Qualifications:

A person who has a master’s or doctoral degree in speech-language pathology and who meets either of the following requirements:

  1. Licensed as a speech-language pathologist in the state in which the individual furnishes services.
  2. In the case that the individual provides services in a state that does not license speech-language pathologists, has successfully completed 350 clock hours of supervised clinical practicum (or in the process of accumulating supervised clinical experience) AND performed not less than 9 months of supervised full-time speech-language pathology services after obtaining a masters or doctoral degree in speech-language pathology or a related field AND has successfully completed a national examination in speech-language pathology approved by the secretary.
  3. Must have a criminal background check.
  4. Must have current CPR certification

Responsibilities:

  • Receives the intake referral information and prepares paperwork/tools necessary for the visit.
  • Obtains all pertinent medical history from Client, family or significant others.
  • Evaluates Client’s speech and language abilities, both defects and assets and performs periodic re-evaluation.
  • Plans and provides rehabilitative services for speech and language disorders.
  • Records type of treatments and Client’s response on clinical progress notes which are written on the day of the visit and incorporated into the Client’s medical record within 7 days.
  • Assists the physician in evaluating the level of function.
  • Helps develop an appropriate and effective ST Plan of Care (POC) to be submitted to the physician for approval and implementation, revising as necessary and at least every 30 days.
  • Functionally reassess the Client for on-going therapy.
  • Determines medical necessity for other services for the case. Selects and administers diagnostic and therapeutic ST techniques and materials.
  • Effectively communicates with Client and family the POC and progression of such.
  • Effectively communicates with other disciplines in the case (case conferencing).
  • Communicates effectively with the Director of Skilled Services/Nursing Supervisor scheduled visits planned and changes to the schedule on a weekly basis.
  • Instructs and counsels other health team personnel and family members in methods of assisting Client in improving, correcting and accepting his/her speech disabilities.
  • Communication with the Client’s physician (verbally and/or in writing) to obtain effective ST treatment modalities and/or rehabilitative therapy modalities to effect the best means to obtain the desired outcome for the specific medical problem that caused the case to open and/or recertify.

Job Description: The Clinical Manager is a qualified professional who is approved by the Board of Directors at the recommendation of the Administrator who is responsible for the assessment, coordination,
supervision and evaluation of the designated clinical activities.

Qualifications:

  • A person who is a physician, Physical Therapist, Speech-Language Pathologist, Occupational Therapist, Audiologist, Social Worker or RN.
  • A current license in their field of practice in the state the services are provided.
  • Will have a minimum of two (2) years of clinical experience, one (1) of which has been in a supervisory or administrative role in a home health care setting.

Responsibilities:

  • Provides superior customer service to internal and external clients, customers and clients
  • Accountable for the care and outcomes that his/her staff is providing
  • Meets weekly with clinicians to review client caseloads, assessing the quality of care, coordination of all disciplines involved and appropriate allocation of services
  • Mentors new clinicians
  • Performs joint visits with clinicians as a means of assessing clinician ability to manage client care needs, leading to excellent client outcomes and education of clinician in accurate assessment and documentation of assessment utilizing current assessment tool

Makes cold calls to all potential healthcare settings that could use our services:

  • Analyzes Client/company relationships.

Marketing Plan:
Develops a marketing plan, identifying priorities and a reasonable timetable. Implements marketing plan staying within established timetable. Reviews and evaluates the analyses and plan on an established basis. Participation in the Agency QI Program

    Association, Inc. (AOTA) or its successor organizations.
  • Is eligible to take or successfully completed the entry-level certification examination for OTAs developed and administered by the National Board for Certification in Occupational Therapy, Inc. (NBCOT).
  • A minimum of two (2) years experience as an OTA, preferably in the homecare setting.
  • Current CPR certification.
  • If educated outside the United States, on or after January 1, 2008—
    Graduated after successful completion of an occupational therapy assistant education program that is accredited as substantially equivalent to occupational therapist assistant entry level education in the United States by:
  1. The Accreditation Council for Occupational Therapy Education (ACOTE).
  2. Its successor organizations.
  3. The World Federation of Occupational Therapists.
  4. By a credentialing body approved by the American Occupational Therapy Association

Job Description: Under the supervision of the Physical Therapist and according to established treatment plans and specific procedures, provides treatment to physical therapy Clients in order to assist them in regaining physical mobility and function, increase strength, and decrease pain.

Qualifications:

A person who is licensed, registered or certified as a PTA, if applicable, by the state in which practicing, unless licensure does not apply, and meets the following requirements:

  • Graduated from a physical therapist assistant curriculum approved by the Commission on Accreditation in Physical Therapy Education of the American Physical Therapy Association.
  • Current PTA license in the state of practice.
  • A minimum of two (2) years experience as a PTA. preferably in home health care.

If educated outside the United States or trained in the United States military, graduated from an education program determined to be substantially equivalent to PTA entry level education in the United States by a credentials evaluation organization approved by the American Physical Therapy Association or identified at 8 CFR 212.15(e)

  1. Passed a national examination for physical therapist assistants.
  2. Has a current PTA license in the state of practice.

Responsibilities:

  • Assists PT in the implementation of individual Client physical therapy care plans; performs daily physical therapy assignments to accomplish
  • Client therapy plans and treatment goals.
  • Responsible for the daily care of assigned Clients per Client POC.
  • Guides Clients through prescribed exercise programs, including passive, assistive, and resistive exercise routines designed to develop or enhance strength, muscle tone, and range of motion.
  • May participate in the development of home exercise programs designed to meet individualized Client needs.
  • Applies heat, light, water, and electrically based treatments/modalities (i.e., ultrasound, infra-red lamp, muscle stimulation, traction, hot and cold packs, paraffin baths, whirlpool, diathermy, etc.) to stimulate
    muscle/nerve function.
  • Assists Clients in gait training (i.e., the use of parallel bars, walkers, crutches, canes, braces, etc.) in order to improve ambulatory skills.
    Performs other therapeutic activities such as massage and body mechanics instruction.
  • Observes Client condition during treatment and notes any significant change in Client condition.
  • Seeks assistance from PT or medical personnel when appropriate.
  • Records Client response to treatment and overall progress in Client charts.
  • Refers Clients to PT for reevaluation as necessary.
  • Prepares Client referrals as requested by PT.
  • Advises professional team members of Client progress and perceived effectiveness of treatments; may suggest revision of treatment plan to better meet Client needs and capabilities.
  • Escorts/transports Clients to physical therapy area, giving verbal guidance to maximal physical assistance as required; obtains wheelchairs, carts, walkers, etc. and assists Clients into chairs and onto
    carts and table as necessary and provides physical support at all times.
  • Prepares physical therapy equipment.
  • Cleans and maintains therapy equipment by cleaning and changing mats, changing linen, cleaning and refilling equipment, etc.
  • Follows sterile procedures and guidelines and ensures that all equipment is working properly and reports needed repairs to supervisor.
  • Assists Physical Therapist in coordinating Client load. Participated in educating the Client and family.
  • Prepares charges for services provided and enter charges into computer if required.
  • Remains abreast of developments in the field of physical therapy technology by pursuing a program of self-development, attending in service and continuing education programs, etc.
  • Participates in in-service programs.
  • Participates in QI program.
  • Performs related duties as required.

Job Description: Audit and review of all client charts (Medicare, Medicaid, HMO), record reviews and examination of visit notes for compliance and completeness. Comprehensive knowledge of Quality Assurance processes in a healthcare field is preferred.

Qualifications:

  • A registered nurse and a minimum of two (2) years home health experience one of which should include experience as a QI nurse of a Medicare certified home health agency.
  • RN license must be current in VA.
  • Must have a working knowledge of Medicare home health requirements, good organizational, interpersonal and judgment skills.
  • Must be able to develop action plans and resolve difficulties.
  • Must have a criminal background check.

Responsibilities:

  • Review of all Medicare, Medicaid and HMO client charts and files to assure proper documentation to maximize reimbursement.

  • Transfers a lesser compliance rating of 80% or below to the QI Committee for action plan formulation. Reviews and edits all OASIS documentation for accuracy and absence of errors. Works with SN’s

  • Clinical Record Review including: verification of all documentation, quality of care provided, visit utilization, adherence to the care plan, evidence of communication between all disciplines.
  • Communication and collaboration with physicians regarding clients.
  • Charts are to be audited for accuracy and completion per Medicare guidelines.
  • Assist with training programs and development of materials to resolve issues.
  • Communicate with the Governing Body of situations requiring extensive training or action.
  • Monitor denial notices (ADR/TPL) and respond with additional information.
  • Notify the Governing Body of trends.

Job Description: Provision of skilled respiratory therapy service on an intermittent basis to clients in their residences in accordance with MD orders and a written plan of care.

Qualifications:

  • A graduate of an AMA approved RT Program and have minimum license of CRT or RRT
  • Be licensed in VA.
  • Two (2) years’ experience in providing respiratory care in an acute or home health setting.
  • Criminal Background check.

Responsibilities:

  • Comprehensive assessment, initial & ongoing, of the patient’s condition/level of function/home environment to determine appropriateness of care/equipment provided.
  • Coordination with MD/Agency staff in development of the interdisciplinary care plan as it pertains to respiratory therapy.
  • Provision of prescribed equipment and instruction of client/family/agency staff (as appropriate) in care and use of respiratory equipment.
  • Consultation/training/technical assist to patients/caregiver/agency staff regarding use of respiratory equipment, supplies and techniques.
  • Identifies additional service needs and coordinates services as needed.
  • Oversight of continued effectiveness, safety and appropriateness of the respiratory equipment.

Reporting employee issues including but not limited to: no call no show, unreliability, failure to follow the schedule, changes days/times of visits without notification are critical to the success of the department’s standards.

Qualifications:

  • High School graduate (or equivalent).
  • A minimum of one (1) year recent experience as a scheduling coordinator in a home care agency or other healthcare field.
  • Ability multi-task and a propensity for working with and helping people

Responsibilities:

  • Takes incoming referrals timely.
  • Interacts with contract case managers and others to enhance the PR of the agency.

  • Prints and distributes weekly accurate para schedules.
  • Reports paras who service Clients outside the scheduled times without notification of the office.
  • Receives and follows up on faxes received from answering services each morning.
  • Takes and gives report from/to On-Call HA each morning.
  • Reports all complaints/problems related to off hour issues (answering service) or On-call.
  • Participates in the QI Committee.
    Participates in the orientation process as requested.
  • Cooperates with Scheduling Manager regarding scheduling projects as designated by the Administrator to improve internal scheduling processes.
  • Opens all certified HA requests within 48 hours of receipt
  • Reviews para paperwork at the end of every week for accuracy and completeness so that all medical records are intact and up to date.
  • Enforces the Agency’s disciplinary process without prejudice or discrimination.
  • Participates in in-service programs.
  • Notifies the Agency of emergencies, sickness, and other imminent occurrences that may affect the Client caseload as quickly as possible relative to the event’s occurrence.
  • Submits written time requests 2 weeks or more in advance of planned time off.
